Home Care of Persons Receiving Continuous Ambulatory Peritoneal Dialysis

Abstract
CAPD is convenient and feasible treatment alterative to hospitalization. However the needs for Home Nursing care, particularly related to patient and family educational, general nutrition, CAPD management and hygiene all which contribute to the prevention of peritonitis and pericatheter infection, have not been investigated. This study explored need for home visiting nursing care of persons receiving CAPD and anticipating the necessity of life-long treatment. The purpose was to contribute to the planning of home visiting nursing services for these clients, and thus to the promotion of a satisfying life style. The study subjects were a convenience persons receiving CAPD at home and attending out-patients Renal clinics at general hospitals in Seoul, twelve patient concerned to an interview at home and nine preferred being interviewed at the clinic. Date were collected from Sept 5th to 26th, 1991 by a structured questionnaire, and through direct observations and personal interviews. The needs were classified into six categories reflecting both the grounded data and the literature. Needs for Home Visiting Nursing Care : 1. Management of CAPD Patients¡¯ needs related to the management of CAPD focus on problems of infection due to the failure to use knowledge of CAPD and the lack of knowledge of CAPD. Failure to use knowledge is related to problems of disinfection of the connecting points of catheter and hand, putting on masks, and the occupational environment. Lack of knowledge is relevant to the necessity of disinfection of the connecting points of the catheter, the methods of disinfection of hands and the exchanging method. The is need for understanding of rational behind management procedure and reinforcement about aseptic techniques. 2. Nutrition. Nutritional needs are related to nutritional deficiencies due to lack of money, the failure to use knowledge especially for women, lack of knowledge of dietary control and its importance and physical complications such as nausea, vomiting, and poor appetite. Education of patients receiving CAPD and their families about the importance of the dietary control and specific guideline corresponding to the is standard of living and knowledge is critical. 3. Personal and Environment Hygiene There are two needs related to personal hygiene and environment. They are the failure to use knowledge and the lack of knowledge about ventilation and bathing. Education of patients receiving CAPD about the importance of personal hygiene and the environment and ways to meet these needs in indicated. 4. Control of Blood Pressure and Weight. The control of the blood pressure and the body weight are also related to the failure to act on knowledge and the lack of knowledge, specially the lack of awareness of the necessity of control of blood pressure and weight, lack of knowledge of weight control methods and the use of the sphygmomanometer and scales. Nursing approached to these findings will need to be creative. 5. Emotion Support Patients receiving CAPD express feeling of helplessness, despair, incompetence and lowered self-esteem related to changed body image, they need emotional supports. 6. Patient Education Educational needs of patient receiving CAPD are related to the appropriate timing, methods, frequency, content, and patient evaluation. Systematized educational programs and continuing educations required. These findings contribute to the planning of nursing guidelines specific to clients receiving CAPD and thus to the enhancement of valued life styles and the prevention of frequent hospitalizations.
